Chicago Man Faces Charges Of Prescription Fraud In Kosciusko County



A Chicago man currently incarcerated in Whitley County faces charges of forgery and fraud in Kosciusko County.
According to the probable cause affidavit provided by Kosciusko County officials, Warsaw police officer Philip Hawks was called in to Walgreens pharmacy on June 18. A Walgreens employee told Hawks that a male subject attempted to fill two medications with one prescription, which is unusual. 
The prescription was for a controlled substance and a legend drug. The employee told the man, Cordell S. Williams, 57, Chicago, Ill., that she would check the prescription before filling it. Williams took the prescription back from her and left Walgreens. 
The employee said that she then called the local CVS pharmacy to warn them about Williams, who had allegedly already been there. Hawks spoke with the CVS employee who said there were at least five other things wrong with the prescription Williams provided. 
The employee told Williams that he would not fill the prescription and then kept it.
The prescription was from a “Grove Medical Center” with a “Christopher Doran” signature on the bottom. The CVS employee told Hawks that Williams had attempted to use the prescription at a Columbia City CVS and that he had been arrested. 
Williams refused to speak with Warsaw police.
