An Elkhart plastics and pallet recycling company is facing $17,000 in fines from I-OSHA after an employee died from an explosion. The blast happened at Green Stream on December 31, 2014. OSHA concluded its investigation, documenting several workplace violations, according to our reporting partners at The Elkhart Truth. The violations included no fire extinguishers in place where cutting performed with a torch was taking place, extinguishers that weren't visually inspected on a monthly basis and no educational programs for workers on how to use fire extinguishers. 51 year-old Larry Sanders died of burns at a Fort Wayne hospital hours after the explosion.
