Kosciusko County officials have released the names of the drivers involved in a two-vehicle fatality collision that occurred Monday, east of Leesburg, according to 1st Sgt. Chad Hill, KCSD Public Information Officer.
Kosciusko County Sheriff’s Department Deputies, along emergency personnel, responded to the intersection of CR 700N and CR 200E at 12:34 p.m. Monday, after being notified a vehicular crash with one individual ejected onto the roadway.
Officers arrived and summoned the Kosciusko County Coroner’s Office, who pronounced David E. Holbrook, 70, of Warsaw, deceased at the scene.
The driver of the second vehicle, identified as Wayne L. Sholtey, 74, of Syracuse, was air-lifted from the scene and taken to Lutheran Hospital in Fort Wayne in serious condition.
A preliminary investigation of the crash by Kosciusko County F.A.C.T. team reconstructionist determined that Holbrook had been travelling southbound on CR 200E and failed to stop at the intersection of CR 700.
Holbrook’s 2002 Ford pickup truck struck the passenger’s side of Sholtey’s 1988 Freightliner grain truck, causing it to roll-over and come to rest on its side.
The Kosciusko County F.A.C.T. team was assisted by the Leesburg/Plain Township Fire Department, Lutheran Kosciusko E.M.S., and the Kosciusko County Sheriff’s Department.
