One dead, several injured in crash northwest of Warsaw

WARSAW — A head-on collision left one man dead and four others taken to hospitals Saturday afternoon.

The accident happened on CR 150W, south of CR 400N in Plain Township at 5:35 p.m., according to information provided by the Kosciusko County Sheriff’s Office.

The crash involved a 2024 Tesla driven by Anish Daita and a 2007 Chevrolet driven by Joseph Criswell, 44, of Milford.

Police determined that Daita was northbound on CR 150W was left of center and struck the vehicle driven by Criswell whose vehicle was southbound on 150W. 

Criswell was pronounced deceased at the scene by the Kosciusko County Coroner’s Office.

Daita, 31, of Warsaw, suffered internal injuries and was transported to Lutheran Fort Wayne. 

Two other adult passengers in the Tesla were treated and released at the scene suffering from minor lacerations and bruising.

Six juvenile passengers in the Chevrolet were treated for varying injuries. Two female juveniles were flown to Fort Wayne Lutheran for treatment of their injuries. One juvenile female was ground transported to Lutheran and two male juveniles were treated at the scene for minor injuries. Another juvenile female was treated for injuries at the scene.

The Kosciusko County Sheriff’s Office was assisted at the scene by the Warsaw Police Department, Leesburg Fire Department, Lutheran ground and air ambulance, Kosciusko County Coroner’s Office, North Webster EMS, and Crouse Wrecker Service.
